How did you extract the archive? It doesn't seem to be correct.

download pluginmanager-0.4.0.tar.gz file unzip

put folder in C:\AppServ\www\mantisbt-1.1.8\pluginmanager\

open browser link localhost/mantisbt-1.1.8/pluginmanager/plugins_install_page.php

Mantis: 1.1.8
PHP: 5.2.6
SQL: MySQL 5.0.51b
OS: Windows 7 x86

The archive should be extracted in the mantis root directory not under a pluginmanager sub-directory as you did.
